Prenatal Genetic Testing May Detect Maternal Cancer

Currently, noninvasive prenatal genetic testing is widely available to test for chromosomal abnormalities such as Down syndrome and trisomy 18. A new study has found that the procedure may have a benefit for the mother: it may detect an early stage cancer. The study was published online on June 5 in the journal JAMA Oncology by Belgian researchers.

Genetic Tests For Breast Cancer


The study authors note that noninvasive prenatal testing for fetal genetic abnormalities is focused primarily on the detection of the most common aneuploidies, trisomies 21 (Down syndrome), 18, and 13. However, gene sequencing not only detects fetal trisomies (an extra chromosome) but also other chromosomal abnormalities in the fetus. They explain that the application of a similar gene sequencing approach to DNA in the blood plasma of cancer patients has recently been shown to detect tumor-associated information. Therefore, they developed a gene sequencing procedure that not only checked for the common trisomies but also maternal genetic abnormalities.

The researchers found that during noninvasive prenatal testing of more than 4,000 pregnancies by concurrent sequencing of maternal plasma DNA, three abnormal genome profiles were found. A maternal cancer was suspected, and those three patients were referred for whole-body magnetic resonance imaging (MRI). The MRIs revealed an ovarian carcinoma, a follicular lymphoma, and a Hodgkin lymphoma; each of these malignancies was confirmed by subsequent pathology and genetic assessments.

The authors concluded that maternal DNA sequencing for noninvasive prenatal testing may also provide accurate presymptomatic detection of maternal cancers. They noted that more studies are needed to validate their findings; however, they are confident they are closer to an effective, noninvasive diagnostic technique for early-stage cancers.

n a press release, principal investigator professor Joris Vermeesch, explained: “During pregnancy, cancer-related symptoms may well be masked; fatigue, nausea, abdominal pain, and vaginal blood loss are easily interpretable as a normal part of being pregnant. NIPT [noninvasive prenatal testing] offers an opportunity for the accurate screening of high-risk women for cancer, allowing us to overcome the challenge of early diagnosis in pregnant women.” Dr. Vermeesch is head of the laboratory for cytogenetics and genome research at Katholieke Universiteit Leuven–University of Leuven, University Hospitals, Leuven, Belgium.

Pre-birth Exposure to DDT Linked to Breast Cancer Risk

Women who were exposed to high levels of the pesticide DDT while in the womb are at a four-fold greater risk of developing breast cancer than those exposed to lower levels, according to a new study. The research, published June 16 in the Journal of Clinical Endocrinology and Metabolism (JCEM), found evidence of a link between women exposed to DDT during their pregnancy and the risk of breast cancer to their daughters.

Environmental chemicals have long been suspected as causes of breast cancer, but until now, there have been few human studies to support this idea,” study author Barbara A. Cohn, PhD, director of Child Health and Development Studies at the Public Health Institute in Berkeley, Calif., said in a news release.

Banned in the U.S. since 1972, DDT was one of the first pesticides to be recognized as an endocrine disruptor – a group of chemicals that interfere with the body’s endocrine system and produce adverse developmental, reproductive, neurological, and immune effects in both humans and wildlife. Previous studies found that exposure to DDT was associated with birth defects, reduced fertility and an increased risk of type 2 diabetes, but links to breast cancer were weak -- until now.

For the new study, Cohn and her colleagues tracked the daughters of women who participated in the Child Health and Development Studies (CHDS) from 1959 to 1967. At that time, DDT was a widely used pesticide that accumulated in the fat of animals, milk, butter, cheese and other products in the food supply. It was also present in a number of products found in the home.

The CHDS research involved 20,754 pregnant women who gave birth to 9,300 daughters during that period. The researchers used state records and a survey of CHDS participants’ grown daughters to uncover how many had been diagnosed with breast cancer by the time they turned 52.

To determine the levels of DDT the daughters were exposed to in the womb, the research team analyzed stored blood samples that were taken from their mothers during pregnancy or shortly after they delivered their babies. During the 54-year follow-up period, the researchers measured DDT levels in the mothers of 118 women who were diagnosed with breast cancer and compared them to DDT levels in 354 women from the group who did not develop breast cancer.

Cohn and her colleagues found that elevated levels of DDT in the mother’s blood were associated with a 3.7 times higher risk of breast cancer in their daughters, than those exposed to lower levels. They also found the higher the level of DDT in a mother’s blood, the more likely her daughter would be diagnosed with more advanced breast cancer.

The link to DDT remained strong even after accounting for the mother’s breast cancer history. Among the women diagnosed with breast cancer, 83 percent had the estrogen-receptor positive form, where the cancer receives signals form the hormone estrogen to promote tumor growth. Because DDT has been found to interfere with the production of estrogen, this finding may explain the link.

Although the authors see their study as the first to provide evidence of an association between DDT and breast cancer, they acknowledge that it does not prove a cause-and-effect. However, with the women most heavily exposed while in the womb in the 1960s currently reaching the age of a heightened risk of breast cancer, the researchers see a need for a new emphasis on finding and controlling environmental causes of breast cancer that operates in utero.

Our findings should prompt additional clinical and laboratory studies that can lead to prevention, early detection and treatment of DDT-associated breast cancer in the many generations of women who were exposed in the womb,” Cohn said in the news release.

Two other hormones important to bone health also are under seige from alcohol. For women, alcohol decreases estrogen. As estrogen drops, it leads to bone loss. In the menopausal years, this adds to the bone loss that's naturally occurring. The stress hormone cortisol – a key topic in the alcoholism relapse book Every Silver Lining Has a Cloud – is found at a higher baseline in those of us with the disease of alcoholism, and remains high even after quitting. High levels of cortisol decrease bone formation and increase bone breakdown.

Johnson, who is assistant director of the Center for Advancing Exercise and Nutrition Research on Aging at Florida State University, says the findings from this study suggest eating blueberries could potentially delay the progression of prehypertension to hypertension, therefore lowering cardiovascular disease risk.

Johnson says she is interested in investigating how functional foods, which are foods that have a positive impact on health that goes beyond basic nutrition, can help to prevent and reverse negative health outcomes. She has a particular interest in exploring this in postmenopausal women. Johnson points out that cardiovascular disease is the leading cause of death in the United States. Women are at greater risk for cardiovascular disease once they go through menopause. The addition of blueberries to the diet may help to lessen the negative cardiovascular effects which are often seen as a result of menopause.

In this study after eight weeks participants who received blueberry powder versus placebo powder with their meals on average had a 7 mmHg or 5.1 percent, decrease in systolic blood pressure. These participants also saw a 5 mmHg, or 6.3 percent, decrease in diastolic blood pressure. Furthermore, participants in the blueberry treated group had an average decrease of 97 cm/second, or 6.5 percent, in arterial stiffness.

It was also observed that nitric oxide, which is a blood biomarker known to be involved in the widening of blood vessels, increased by 68.5 percent in the particpants who consumed blueberry powder. This is significant because arterial stiffness and the narrowing of blood vessels are both seen with hypertension. A rise in nitric oxide is associated with reductions in blood pressure.

This study has been published in the Journal of the Academy of Nutrition and Dietetics.
